Adding Conditional Fields to Forms

When setting up a form template, you can add conditional fields to the form. Conditional fields allow your forms to automatically adapt to each client or staff member as the form is being filled out.

For example, if a client selects Yes when asked if they have any allergies, a text field will appear where they can enter their allergies. If they select No, the text field will not appear.

You can also set up conditional fields that appear automatically for specific clients only. For example, you can show certain fields only to clients of a specific gender, clients who have received specific services in the past, etc.

To add conditional fields to forms, you must be an Admin user, or you must have the Can manage form templates permission enabled in Staff Member settings.

Open the Forms app and select Manage templates.

Select the form template and go to the Form Fields tab.

Note:

If you need to add conditional fields to an existing form template, you can duplicate the form template and add the fields to the duplicate. This way, the duplicate won't be visible until you're done adding all the fields and you've enabled it. Once you've enabled the duplicate, you can delete the previous form template.

Add a new form field, or select an existing form field. Enable the Show conditionally toggle.

Enter an optional Condition name for the conditional field. This name will not be visible when filling out the form. It will only be visible when previewing the form template. For example, a name such as Only shows when they have allergies can help you see at a glance when the conditional field will appear.

Select Add condition and choose the condition that will control when this field appears. The field will be hidden until this condition is met.

You can select from two different condition types:

  • Form field response: This option allows the field to appear based on what the client or staff member selected or entered in any previous field on the form. If there are no previous fields where the client or staff member can make a selection or enter information, this option will not be available.

  • Client conditions: These options allow the field to appear automatically based on specific client criteria, such as client tags, appointment history, or membership status.

    • These options are not available for external forms—only the Form field response option will be available. This means that for external forms, the Show conditionally toggle may not be available if there are no previous fields on the form where the client or staff member can make a selection or enter information.

The table below outlines all the available client conditions.

If needed, select Enable to enable the form template.
