Introducing Integrated Forms
Keeping track of documents has always been a bit of a chore. Paper forms tend to get lost in the shuffle and digital documentation usually requires a third-party app integration.
Today, we’re excited to announce Integrated Forms, a brand-new feature that enables you to template, customize, and distribute forms to clients, staff, and even visitors to your website—all within Mangomint.
Whether it’s an intake form for a facial, a waiver for a chemical service, or internal SOAP notes, Integrated Forms lets you collect important information and store it directly in the client profile, no third-party app necessary.

Specify which services and service categories you need forms filled out for and how often you want to send them. Want to ensure you send a new client form only once? Easy. Need to refresh your client's record for each laser treatment? No problem.

Accessing your forms is easy — once a form has been submitted, you can view it at any time from the calendar or the client timeline.
Even if your client forgets to fill out their form before their appointment, you can simply check them into the Waiting Room and resend a link directly to their phone or have them fill it out on an iPad or Android tablet.

You can create as many templates as you like and preview them at any time. To activate and send forms, add Integrated Forms to your subscription.
Integrated Forms is available as an add-on on all plans for $50 per month. For businesses with more than one location, each additional location is priced at $25 per month.
